Steps to Enroll
If you need to enroll a new provider, you can contact the Enrollment clerk at 800 333-1081. To provide the necessary information, please have the case name/number, the new provider's legal name, address, phone number, date of birth, Social Security number, email address, and the date the provider started working for the client.
Enrollment forms will be sent to providers via DocuSign or U.S. Mail. These forms will include the provider number. Using the provider number, the provider should enroll in the online timesheet portal.
Additionally, a 426a form (Recipient Designation of the Provider) will be sent to the client/consumer.
A link will be sent to the provider to attend the online orientation. Once orientation videos are completed the new provider will self-schedule the in-person orientation. The provider must bring an unexpired Identification and a Social Security card to the in-person orientation. No copies will be accepted (please arrive early for parking, space is limited).
After the required documents, identification and social security cards are submitted, and both orientations are complete, the provider can view timesheets online at etimesheets.ihss.ca.gov
